Registration Procedures

Welcome to Trinity Lutheran Preschool! We require the completion of several forms for each child enrolled in preschool. We appreciate your cooperation in completing and returning these forms for your child’s file. It is required by the State of Colorado that all the information on all these forms be collected and kept in the file at the preschool. Please complete all the forms attached and bring to registration.

A non-refundable registration fee of $45.00 / $55.00 family is required in order for your child to be registered for the up coming school year. We also require that each child has a t-shirt that they wear on field trips, the t-shirt fee is an additional $12.00.

All completed forms are required for your child’s attendance on the first day of school. Besides the completed paperwork there will also need to be a completed physical signed by your child’s doctor and a copy of immunizations.
